The UNDERSIGNED, (hereinafter call “Applicant”) hereby applies for membership in and agrees to purchase electric energy from Northfork Electric Cooperative, Inc. (hereinafter called the “Cooperative”) upon the following terms and conditions.
- The Applicant will, when electric energy becomes available, purchase from the Cooperative all electric energy used on the premises described above and pay monthly the Cooperative billings for energy and services.
- The Applicant agrees to comply with and be bound by the Cooperative Articles of Incorporation, By-Laws and Terms and Conditions of Service, as amended.
- The Applicant agrees that:
a. This application, once accepted by the Cooperative, is a binding contract obligating the Cooperative to provide electric service and obligates Applicant to purchase electricity from Cooperative;
b. The premises receiving service will be wired in accordance with applicable codes and Cooperative code and specification requirements;
c. The materials and electrical facilities installed shall be and remain Cooperative property and may be removed upon termination of this agreement;
d. Upon request, if Applicant is owner of the property, they will grant right of way to easements over, across and/or under lands owned for construction, operation, maintenance or relocation of electrical lines and facilities used to furnish electricity to the Applicant or other members of the Cooperative;
e. Applicant will not permit anyone to disconnect or interfere with Cooperative lines or facilities;
f. Provide an acceptable site to the Cooperative for the meter connect and allow access for ingress and egress to maintain the electric service lines and facilities;
g. Allow right of way clearing to cut, trim and chemically treat trees and shrubbery that may interfere with or threaten to endanger the Cooperative lines, equipment or facilities;
h. A portion of the amount paid for electric service each year is for a subscription to OKLAHOMA LIVING magazine ($5.64), a monthly publication of the Oklahoma Association of Electric Cooperatives and for a subscription to SPARKS & FLASHES, a monthly newsletter of the Cooperative, for members of the Cooperative.
4. The person(s) signing this Application expressly warrant that they have the authority to sign for or on behalf of the Applicant.
